Our Hospice Social Workers build relationships with patients and their families as part of a multidisciplinary team providing competent and compassionate end of life care. Qualifications

 


Has a Master of Social Work (MSW) degree from a school of social work accredited by the Council on Social Work Education. Current NM Social Work license at LMSW or LISW/LCSW level. Some positions require an achieved state licensure of LISW in New Mexico.
2. Prefer 2-3 years of Social Worker experience in Homecare/Hospital/Hospice setting

Responsibilities

 


*Assesses patient and family psychosocial status, emotional factors related to illness including bereavement, environmental resources and obstacles to maintaining safety, response to interventions, family dynamics and communication patterns, potential for risk of suicide and/or abuse and neglect.

*Participates in effective interdisciplinary care coordination, identification of problems, development of plan of care, adherence to plan and alterations and discharge planning when appropriate. Identifies and utilizes appropriate community resources. Assesses patient and family ability to access services. Works as liaison to inpatient or outpatient care coordinators, community hospice resources or facilities, chaplains, others as needed.

*Offers short term individual and family counseling, community resource planning and crisis intervention when necessary.

*Assists other team members in understanding social, ethical and emotional factors related to health problems.

*Assesses special needs related to cultural diversity including communication, space, role of family members, special traditions, especially those related to illness or end of life.

*Assists patient/family in accessing financial resources when necessary, i.e., charity care or patient care funds.

*Identifies obstacles to compliance and assists in understanding goals of care. Involves patient and/or family in treatment plan

*Provides information and assist with advanced directives when appropriate.

*Records in timely manner evaluation data, progress notes and interventions with patient/family and their responses in electronic medical record (McKesson and EPIC).

*Participates in quality and performance improvement measures.

*Supervises social work students in practicum.

*Participates in social work peer review, peer field observations and in-services as appropriate.

*Keep updated with email calendar on regular basis.

*Meet established productivity standards

*Complies with standard precautions, infection control and safety procedures.

*Honors confidentiality as spelled out by HIPPA and Presbyterian
