Summary:

Presbyterian is hiring an RN Care Manager for the Las Estancias Family Medicine Clinic.  The Care Manager pro-actively manages the care for patients along a continuum by facilitating collaborative care between patient, family/support system, physicians, and other ancillary departments/members of the primary care team

Credentials: Registered Nurse - State of New Mexico or Compact State Nursing License required.

 

1-3 years ambulatory experience

 

BLS certification required at time of hire.

 

Education: associate degree in nursing
