Overview

Now hiring a full time Community Health Worker!


Under indirect supervision, provides patient support in the ambulatory care setting according to established standards of care in collaboration with Care Managers, Medical Providers and other members of the health care team. In addition, connecting patient to community-based programs to improve their health and general well-being through education and provision of coordination of care and services.

Ideal Candidate:

High school diploma or GED equivalent

One year of experience preferred interacting with community members or in community/public health services.

Qualifications

  • High school diploma or GED equivalent
  • One year of experience preferred interacting with community members or in community/public health services.
  • CHW Certification required as training is available.
  • Must be able to speak language of the population served.


Skills

  • Skill in use of computers and related software applications, including e-mail.
  • Ability to communicate and set boundaries.
  • Knowledge of barriers to care that may be encountered by patient.
  • Knowledge of community agencies and resources.
  • Skill in time management.
  • Strong interpersonal and communication skills and the ability to work effectively with a wide range of stakeholders in a diverse community.


    Working Conditions
  • Comfortable working in an Emergency Department and/or clinical setting, including Behavioral Health.
  • Can work in both clinical and community-based settings, including clients homes.


Education Essential: High School Diploma or GED

 

 

Responsibilities

  • Participates in presentations to clinics and hospital staff
  • Seeks and completes training opportunities directly related to CHW specialty and/or CHW Core Competency training class to build their subject matter expertise
  • Co-facilitates health and wellness classes, workshops and/or trainings
  • Assists with orientation and mentoring of new team members
  • Participants in local, regional or state community health care worker events/meetings.
  • Screen patients as related to social determinants of health and link patient to community resources that will assist with those needs.
  • Utilizes motivational interviewing and other techniques to engage patients and families in self-management.
  • Acts as an advocate for the patient and family.
  • Educates patients, families, providers, and care team to address social determinants of health.
  • Build relationships with internal and external stakeholders including other CHWs and Peers with the goal of improving the health of the community.
  • Continuously expands knowledge and understanding of community resources and services available.
  • Facilitates patient access to community resources and utilization of these resources, including connecting to social service agencies and assisting with completion of applications.
  • When applicable, initiate closed-loop referral for programs for which patient may be eligible.
  • Works collaboratively and effectively within a team. Establishes positive, supportive relationships with participants and provides feedback to other members of the team. Builds and maintains positive working relationships with the clients, providers, case managers, social workers, agency representatives, supervisors and office staff, from diverse cultural and socio-economic backgrounds. Works to reduce cultural and socio-economic barriers between clients and institutions.
  • Performs other functions as required.
