NOW HIRING A HIM Document Analyst
In order to allow for timely completion of the medical record, the Chart Completion Analyst will review each discharged patients record for incomplete or missing items, identify the responsible provider for each of these items, and link the deficiencies to the appropriate physician for completion. They will be responsible for monitoring the reanalysis queue to ensure all records are completed appropriately. The chart completion analyst will be responsible for verifying chart completion and doing the quantitative analysis of each inpatient, Skilled Nursing Facility Unit, ambulatory surgery, and GI lab medical record based upon standards established by JCAHO, CMS and the Medical Staff Rules and Regulations.

Ideal Candidate/Prefer Qualifications:
Two to five years relevant experience, with at least one year of experience, two preferred in an acute care setting with primary responsibility for maintaining document requirements. Demonstrated competency in all areas of Medical Records, especially in the area of medical record completion and discharge analysis. Previous experience with electronic patient record preferred. Good written and communication skills
*Reviews each record for missing documents, missing signatures or missing text according to procedure.
*Performs a quantitative analysis of each medical record to ensure completeness of documentation. Assigns completion of deficient medical records to the appropriate members of the medical staff. Inputs analysis of medical record information into computerized chart tracking system
*Ensures that all inpatient, Skilled Nursing Facility, ambulatory surgery and GI lab medical records are analyzed by reviewing the list with discharged patients names and comparing it to the computer system.
*Provides back up coverage for the prepping, scanning and indexing area and assists physicians with record completion on a one-to-one basis.
*Assists with re-analysis of medical records to ensure completeness and updates the chart tracking computer system. Prepares weekly pre-suspension reports for medical staff leadership.
*Demonstrates a consistent level of performance; maintains documented standards of quality and productivity.
*Operates office equipment used in the routines of daily work, such as photocopy and facsimile equipment, computers and telephones and scanners
