Overview

Inpatient Hospice and Palliative Care is hiring an experience Registered Nurse (RN) to join the Inpatient team.  In this role you will facilitate Hospice and Palliative Care referrals from all sources and obtains reviews & coordinates the necessary information to complete the clinical assessment of the patient for the correct program & correct LOC within that program. The position assumes dual accountability for Palliative Care and Hospice referrals.

Qualifications

  • Associate Degree in Nursing
  • State of New Mexico or Compact State Nursing License
  • BLS certification REQUIRED at time of hire
  • ACLS
  • PALS
  • Minimum three years acute care experience required and hospice or home care experience preferred.
  • Basic typing competency.
  • Obtain Hospice/Palliative RN certification within two years of hire.

Responsibilities

Provides palliative care & hospice services based upon patient assessment, including direct patient care. In all care, utilizes nursing assessment skills to determine presence of symptoms and response to interventions for those symptoms. Assessments include a review of the full spectrum of patient needs, including an understanding of how the physical, psychological/social (behavioral), functional, financial, resource and spiritual needs of patients factor into their condition.

Serves as a consultant to discuss goals of care with the patient/family and communicate those goals to the appropriate care team. In addition, reviews referrals, and delegates the referral to the appropriate hospice/palliative care staff based on patient needs and staff availability.

Performs Hospice consult reviews and coordinates a clinical assessment, determines if patient is appropriate for hospice, communicates with the patient/family about hospice philosophy, criteria, and site of care. Assumes role of hospice RN with hospital hospice patient, including development of hospice plan of care and subsequent rounding, if necessary.

Acts as a clinical resource for all members of the hospice /palliative team. Develops and maintains a working knowledge of all services available within the Home and Transitions Service Line.

Completes comprehensive and accurate documentation timely and consistently that clearly reflects interventions performed.

In all roles, performs as a member of an interdisciplinary team. Assists with orientation and mentoring of new team members as appropriate. Serves as a resource person & professional role model.
