Presbyterian is seeking a Home Health/Hospice & Palliative Scheduling Liason to navigate clinical referrals under the direction of the Patient Care Managers to coordinate Field Clinicians patient visits to meet Regulatory specifications from referring providers. Coordinates staffing/scheduling for all departments ensuring productivity levels for clinicians and Certified Nursing Assistants. Provides Daily operational duties of scheduling related to intake process/workflow. Accuracy in scheduling is paramount in performance. Ability to schedule complex referrals utilizing the Assigned Work Queues; Admissions, Routine visits, Recerts, Resumption of Care, (i.e. post hospital discharges, wounds, total Joint discharges, Nursing visits, Ancillary Services, etc.) Supports the Centralized Agency scheduling tasks for CDS & Regional Agency operations including the Hospice Face to Face Tracking, Benefit Period Tracking, Supervisory visits Reports, Hospice Notice of Elections, Daily Preparation of Hospice Geographic Team Reports.
