Overview

Presbyterian is seeking a Home Health/Hospice & Palliative Scheduling Liason to navigate clinical referrals under the direction of the Patient Care Managers to coordinate Field Clinicians patient visits to meet Regulatory specifications from referring providers. Coordinates staffing/scheduling for all departments ensuring productivity levels for clinicians and Certified Nursing Assistants. Provides Daily operational duties of scheduling related to intake process/workflow. Accuracy in scheduling is paramount in performance. Ability to schedule complex referrals utilizing the Assigned Work Queues; Admissions, Routine visits, Recerts, Resumption of Care, (i.e. post hospital discharges, wounds, total Joint discharges, Nursing visits, Ancillary Services, etc.) Supports the Centralized Agency scheduling tasks for CDS & Regional Agency operations including the Hospice Face to Face Tracking, Benefit Period Tracking, Supervisory visits Reports, Hospice Notice of Elections, Daily Preparation of Hospice Geographic Team Reports.

Qualifications

  • High School Diploma or GED required; Associates degree preferred.
  • Two (2) or more years clinical experience as a Clinical Scheduler required.
  • Short-term training in Medical Terminology
  • Customer service and scheduling experience.
  • Advanced computer skills with Microsoft Office Platform.

Responsibilities

  • Utilization of EMR scheduling application.
  • Performs the patient scheduling process and validates accurate patient demographics.
  • Guide informed dialogue with patients and patient representatives regarding communication they can expect from clinicians. 
  • Serves as the Liaison between the Clinical Field staff; Intake RNS, and Patient Care Managers to provide services ordered.
  • Formulates complete and accurate telephone encounter messages and routes to the appropriate EMR Provider in-basket pool to support operational aspects of patient care.
  • Assigned scheduling for RDS Home Health and Hospice locations as needed.
  • Required to accurately manage the providers block time for visits in CDS and Regional sites.
  • Develop and maintain a positive relationship state- wide agency personnel both in/outside organization.
  • Follow regulatory protocols for scheduling clinicians per CMS guidelines.

    Administrative Duties:
  • Accurately maintains afterhours/weekend pass-down
  • Utilize time and attendance systems for clinician work schedules in reference to patient assignments.
  • Produce various reports as required: Supervisory Visits, Geographic Team Reports
  • Answers Agency Phone Queue, resolves inquiries, routes callers, and take messages as appropriate.
  • Serves as preceptor, trainer, mentor/resource to new employees in CSC department. 
  • Attend Huddles, staff meetings, Employee Forums, participate in PDSA meetings; development of new programs and processes. 
  • Manages and is Responsible for the EMR Schedule Template for all new onboarding clinical staff.

    Financial Accountabilities:
  • Obtains signed orders from providers to ensure timely billing so that payments are processed within regulatory parameters to perform ordered services.
  • Hospice: Navigates the Conditions of Participation Regulation documentation to ensure billing accuracy for reimbursement.

    Quality Improvement/ Patient Safety
  • Appointment reminder calls; coordinates visit confirmation & manages cancellations.
  • Rescheduling-Evaluate clinician schedules and take appropriate action to ensure accuracy and efficiency per regulatory guidelines.
  • Works with assigned clinicians to maximize schedule effectiveness and productivity expectations daily.
  • Willingness to work in a positive manner to assist in development of new programs and processes.
