Overview

Presbyterian is seeking a Senior Program Manager for our Community Health team. This position offers a unique opportunity to lead transformative community health initiatives in Santa Fe, New Mexico—a city known for its cultural richness and deep community ties. You’ll be responsible for designing, developing, and implementing programs that improve population health outcomes for patients, members, and neighborhoods across the region. The role demands strong collaboration with local organizations such as the Santa Fe Public Health Office, Santa Fe Indian Health Center, and Health Care for the Homeless. You’ll guide multi-disciplinary teams that integrate clinical expertise with community insight, navigating real-world barriers like housing insecurity, food access, and transportation challenges. Success in this role requires a systems-thinking mindset, public health knowledge, and the ability to build lasting relationships across sectors. Whether you're launching a mobile health clinic, partnering with local farmers to address nutrition gaps, or co-creating mental health programs with tribal leaders, your work will directly shape the health and well-being of Santa Fe’s diverse communities.

 

Type of Opportunity: Full Time (1.0 FTE/40 hours per week)

This position is eligible for the Northern New Mexico differential of $4.00 per hour. (This differential may be adjusted or discontinued based on business requirements.)

 

 

Qualifications

  • Bachelors degree in public health; community health; public administration, education or related field required. Masters degree preferred, but not required based upon relevant experience. 
  • Seven or more years of relevant industry specific experience demonstrating strong leadership skills and a proven ability to facilitate cross-functional teams.
  • Experience facilitating diverse community-based groups to foster partnerships.
  • Strong research competencies, written and oral communication.
  • Computer knowledge to include Windows, word-processing and database systems.
  • Requires detailed knowledge of population health and public health research and interventions, including the interpretation of data

Responsibilities

  • Responsible for coordinating Presbyterian community health initiatives in Santa Fe

  • Leads implementation and sustainability of relevant community health programs

  • Promotes adoption of a system-wide community health approach across PHS

  • Selects, prioritizes, and redirects initiatives based on evolving needs and impact

  • Develops organizational capacity for innovation and best practice sharing

  • Engages internal stakeholders including PHS executives, service lines, and providers

  • Collaborates with external stakeholders such as network providers and government entities

  • Develops strategies to align practitioners with new community-clinical linkage models

  • Leads provider and community engagement efforts for program adoption

  • Prepares program reports and presents outcomes to senior leadership

  • Represents Presbyterian on community councils and boards

  • Ensures financial and operational goals are met for community health initiatives

  • Identifies grant funding opportunities and develops proposals

  • Tracks and reports on grant deliverables and presents results to leadership

  • Manages the largest and most complex community health programs

  • Oversees programs from planning and design through implementation and evaluation

  • Identifies initiatives aligned with community health needs assessments and collective impact goals

  • Applies integrated decision-making to optimize health outcomes

  • Coordinates outreach strategies and stakeholder engagement for health initiatives

  • Deploys communication strategies to internal and external audiences

  • Performs other duties as assigned to support evolving community health needs
