Overview

Physical Therapist are movement experts who improve quality of life through hands-on care, patient education and prescribed movement. Our Physical therapist work as a  team with physical therapy assistants to treat people of all ages and abilities. This position with Presbyterian’s Home Health department build relationships with patients and their families, working toward the common goal of recovery.  Home healthcare Physical Therapists see the full scope of the patient’s life, from community, to home, to family, and provide personalized care utilizing available resources to meet patient needs. There’s nothing more personal than visiting a patient in their own home.

Qualifications

Less than one year of experience in physical therapy. New graduates are encouraged to apply.

Bachelor's degree required.

Doctor of Physical Therapy Preferred.

NM License of Physical Therapy required.

BLS required at time of hire.

Responsibilities

Requires detailed knowledge of physical therapy in terms of what and how work is to be done as well as why it is done, this level includes interpretation of data.

 

Basic computer skills required.

 

Basic understanding of the rules and regulations of the specific setting

 

Evaluates and treats patients regarding the application of a wide variety of therapeutic interventions for rehabilitation of physical function and developmental disorders.

Must demonstrate knowledge of the principles of growth and development over the life span of patients served

Oversees and insures a comprehensive rehabilitation program designed to meet the physical, psychosocial, and immediate environmental well-being of the patient

Promotes patient/caregiver self-determination by optimizing outside resource utilization to achieve the highest level of independence.

Coordinates and communicates with various disciplines or services involved in patient care.

Coordinates efforts toward discharge goals established at initial evaluation and during episode of care.

Is a patient advocate.

Other duties as assigned.


Participates in educational programs and inservices and other meetings as required. Participates on special project teams and other related duties as assigned and requested
